If I absolutely had to take CoE as canon, a few things had to happen afterwords.

First was that Jack just didn't bugout right after Day Five. He tried to do his job for a few days, but during a routine weevil chase Jack had a complete mental shutdown. Luckily, by that point the weevil had been chased back underground, so no one was hurt. Jack promoted Gwen to active Directorship while he gathered himself back together. Execpt he couldn't, and wasn't even able to give Gwen any advice without having a panic attack.

Gwen was ordered him to take a break and travel. Time and distance didn't help his guilt, his PSTD, or that he thought he was abandoning his post. It also really didn't help that every time he saw a child, he was reminded he killed Stephen. He tried to drink and kill himself to rid himself of the guilt, but after six months he remembered that alcholism didn't solve anything after he lost everything for the second and third time, so it probably wouldn't help this time either.

So Jack ran, and ran, and ran. The third ship he was hitching a ride on was carrying refugees away from a warzone, and Jack remembered that running away didn't help the first time he lost everything, so it probably wouldn't help now.

Jack then decided to try something novel, and ask for help. He couldn't ask the Doctor, because while he knew the Doctor had similar choices and similar griefs, he asking would probably rip open the Doctor's wounds and he couldn't do that to his friend. After a long search, Jack found what he was looking for: A long lived people that hadn't isolated itself from the universe, and that even co-existed with a shorter lived people. And that both groups had soliders.

It took quite a bit of searching. But he found a place, and he's trying to get better. And when he does, Jack will call the Doctor and ask for a lift back to Earth a few days after he left.

Basically, this a very long winded way to say that Jack is off on the other side of the galaxy getting help for his mental problems, and will be back as soon as he can.

And the reason why he has mental problems, because his brain should be reseting back to his normal, is a bit complicated. Firstly, Jack heals just enough that he can live again, then he comes back, and then his body heals back to looking like nothing happened. After he's back to normal, his healing is accelerated, but still takes some time to heal.

I'm proposing that if as soon as Jack comes back, his brain process starts to get off-track again and if the breaking down process outstrips the healing process after the first accelerated bust, Jack can't get better.

Which is how Jack can be more or less okay after The Year that Never Was, but is falling to pieces after Children of Earth.
